Every now and then SL will just decide to stop updating your avatar texture with clothing changes. Refreshing yoru texture does nothing, you're stuck with yoru old texture. This can lead to amusing combinations, like putting on something that requires a full alpha layer and the server going "LOL NO ALPHA FOR YOU!"
This is an older bug. Not sure when I started noticing it.
I have no clue how to reproduce it consistently. I have wild theories but they are still wild theories at this point. I don't even know if it's a server side issue (And outside of viewer control) or a viewer issue. I'll file this JIRA none the less in case anyone else might get more insight or wish to add to it.
Refreshing your avatar texture just causes the old texture that you didn't want to reload.
